Home visits

If you are housebound or unable to attend the surgery due to your health problem and require a home visit, please telephone the surgery on 08444 772564 by 10am. Unfortunately, it is not possible to insist on a particular Doctor for home visits.

Out of hours care

If you need to contact the surgery outside of normal hours, i.e. 6pm to 8am weekdays, during the weekend or bank holidays, our out of hours care is provided by Wiltshire Medical Services. They can be contacted by ringing the surgery on 08444 772564, or on 0300 111 5717.

Sick Certificates

For the first 5 or 6 days you are off work, your absence needs to be covered by a ‘self certification’ note. These can be obtained from your employer or at Patford House reception. This is a legal requirement. If your employer insists on a sickness certificate to cover this initial period of absence, we have to make a charge for this service.

If you have been seen by your Doctor and your illness has lasted over one week, your Doctor will be able to issue you a sick note. If you have been seen or admitted to hospital, ask the hospital to issue a sick note before you leave.

We are unable to backdate sick certificates for time already taken off before we see you. However, repeated sick notes to cover long term illness can occasionally be back dated – please ask reception for details.

Charges for Non-NHS Services

We provide a variety of non-NHS services. Our Doctors will sign passport applications if you have been registered with the Practice for 2 years or more.  We also carry out Private Medicals for registered and un-registered patients and sign most types of insurance form. These services are provided but incur a small charge, so please check with reception for these charges before proceeding with your request.

Patient transport

The patient transport scheme is for patients with a medical need who require extra help to get to hospitals or treatment centres. Information about this scheme is available at reception. The patient transport team are available on 01249 813604 between 8am and 5pm, Monday to Friday.

Calne Link Scheme

Calne Link Scheme is a local voluntary group offering a good neighbour service to local people who are in need either because they are elderly, disabled, single parents or perhaps are temporarily in need because of illness.  Calne Link may be able to assist with transport to medical appointments.  They provide a free confidential service, but welcome donations from their users.  All donations are used for the provision of the service which is run entirely by volunteers.
