How to register

To register at Patford House Surgery, we need you to visit the surgery to collect a registration pack. This pack contains instructions and the forms you need to fill in and return to us.  To allow us to provide you with as much help as you might need, please avoid busy periods wherever possible.

If you are over 16, we will need a urine sample which you will need to give us before 11am.

Change of your personal details

Please remember to let us know if you change any of the following:

• Your name - please come into the surgery to fill in the relevant paperwork
• Your address
• New additions to your family - please remember to bring your FP58 form to the surgery when you register

Temporary residents

If you are staying in the area temporarily, you can still access our services. Please let reception know that you are a temporary resident and they will give you a form to complete. You will need to bring in this form 15 minutes before your appointment.
